Payments are hosted by a third party (Converge) on a Converge hosted site (https://www.convergepay.com/hosted-payments/). Information required for payment on the Converge site include:

  • First Name and Last Name
  • Email
  • Address
  • Phone
  • Credit Card Number, Expiration Date and CVV

Staff of Sparks Family Medicine, Ltd (“SFM”) may contact you regarding payments made on SFM.com. SFM will use your email, phone and/or address from your patient record to contact your regarding your account. If provided for payment, your name, address, email address, the last four digits of your credit card number, the expiration date of your credit card, the payment amount, and the date of payment are retained by Converge for use by SFM.

SFM does not engage in SMS marketing. Should SFM engage in SMS marketing in the future, you will receive an SMS text to opt-in to receiving marketing texts. SFM will not sell or provide your personal information to third parties.

Data Sharing
• Customer data is not shared with 3rd parties for promotional or marketing purposes.
• Mobile opt-in and consent are never shared with anyone for any purpose. Any information sharing that may be mentioned elsewhere in this policy excludes mobile opt-in data.

SFM.com does not allow users to leave comments. If users are allowed to comment on the site, SFM will collect the data shown in the comments form, and also the visitor’s IP address and browser user agent string to help spam detection.

Media
