Insurance Participation

The providers at Sparks Family Medicine are credentialed with participating health plans through Sparks Family Medicine or Dr. Amy Sparks under the EIN/Tax ID of 88-0477868. Please check with your insurance company to see if Sparks Family Medicine/Dr. Sparks are contracted with your health plan. If your provider, Dr. Amy Sparks or Sparks Family Medicine are not listed in your provider manual, our office does not participate with your health insurance plan.

As there are a multitude of health insurance plans for each health insurance company and plans are often contracted through third parties, SFM does not know all of the plans for which SFM may or may not be a participated provider. Please contact your insurance plan to determine if SFM is a participating provider before your first visit and upon changes to your insurance coverage. 

Failure to verify that SFM is a participating provider will result in the patient being responsible for all charges. Failure to use your health plan’s participating providers may limit the coverage available for referrals to laboratory tests, imaging, specialists, etc.
